Coordinating Council for Independent Living
Case Management Services
An Alternative to Nursing Home Care
Caregiving for an older or disabled family member or friend can be physically and emotionally stressful for both persons involved. Although every situation is unique in some way, our trained professionals have experience dealing with the type of problems you may now be facing. The services provided through this program can allow caregivers to take breaks to refresh themselves and maintain their own health.
The earlier you obtain support and assistance, the better able you will be to cope over a long period of time and maintain your own well-being. If you are a caregiver, or if you need assistance yourself, take a positive step of seeking assistance now. Begin by contacting your local Coordinating Council for Independent Living office.
For more information or to make a referral to this program, please contact the Coordinating Council for Independent Living office nearest you and indicate your interest in the Aged and Disabled Medicaid Waiver Program.
The Aged and Disabled Medicaid Waiver Program offers in-home assistance with activities of daily living. This program was designed to allow persons to be cared for in their own homes rather than in long-term nursing homes.
If you or someone you care about requires help with eating, bathing, dressing, grooming, meal preparation, housekeeping or similar tasks, this program may help.
You must be an adult age 18 or over and meet certain medical and financial criteria. Medical criteria will be assessed when CCIL has been notified of your interest in these services. Medical need is based on a person’s ability to care for oneself – not on any particular diagnosis. A West Virginia State representative will travel to your home to conduct these assessments. You need only to call us. Financial eligibility will be determined once medical criteria have been established.
Who Delivers this Service?
Registered Nurses, Case Managers and Homemakers provide a comprehensive package of services. These trained professionals offer expertise, skills, experience and peace of mind that a family might not otherwise be able to access.
The Nurse and Case Manager will conduct assessments, devise plans for your care, and monitor the delivery of direct care services in your home. They will also serve as resource persons for you and your family. The Homemaker will help you with the day-to-day tasks involved in remaining independent such as cooking, bathing, grooming, transportation and housekeeping.
Where Are Services Delivered?
The Nurse, Case Manager and Homemaker will deliver services right in your own home. This allows you and your family to enjoy privacy while receiving services from trained professionals.

Aged and Disabled Medicaid Waiver Program
The Aged and Disabled Medicaid Waiver Program is a federally sponsored, state administered program that waives the regular financial guidelines used to determine Medicaid eligibility. Those who meet the modified requirements are given a Medicaid card which covers the services outlined in this brochure as well as specified medications, physician visits, procedures and hospitalizations.
